What’s a care plan? It’s a roadmap to guide your cancer care following your diagnosis and treatment of cancer. People who have completed treatment of melanoma or are currently in treatment can develop a care plan with their health care team. A care plan can:
- Detail your diagnosis
- Outline the treatments you have had or are currently taking
- Tell you who on your health care team to contact with questions or concerns
- Explain symptoms or side effects you might experience and who to call if you need help managing them
- Outline what you should do and who you should see for follow-up care
