Learn about running the Cancer Transitions® program in your facility. A critical component of Cancer Transitions® is the facilitation by cancer care professionals.
If you have a Cancer Support Community, it is easy to work with them and offer the Cancer Transitions® program. If you don’t, Cancer Support Community offers resources and training for program delivery.
Resources that accompany the program include and online Train-the-Trainer program developed for oncology professionals who will run the program in their office/facility. Cancer Support Community will also provide additional training support for oncology professionals running the program. More information about the Train-the-Trainer program will be coming soon.
