Keeping track of your medical records allows you to share accurate information with all of the health care providers you see after your treatment has ended. In order to maintain this information:

• Ask for and keep copies of all of your medical records

• Bring the records with you when you go to your follow-up visits

• Bring the records when you visit any new medical office (For example: cancer treatment can cause mouth and teeth problems, so your dentist should know about your treatment) 

Try to fill-out the LIVESTRONG™ Cancer Survivor’s Medical Treatment Summary with your health care team (located on the previous page). This document can help you manage and organize the information important to your survivorship experience. If you don’t wish to use this guide, you might find it helpful to keep all records about your treatment and follow-up care in a single binder or notebook.
